概要	A 19-year-old male who is a college wrestler presented with a one-month history of medial elbow pain and numbness involving the ring and little finger. The symptoms worsened with elbow flexion. There was weakness of the adductor pollicis muscle and atrophy of the first dorsal interosseous muscle. The Tinel sign was positive over the retro-condylar groove. A sensory nerve conduction study demonstrated abnormal conduction slowing at 0-3 cm proximal to the cubital tunnel. MRI showed thickening and increased signal intensity of the ulnar nerve on T2-weighted and STIR images at the cubital tunnel. The medial head of the triceps muscle was prominent, and the ulnar nerve was compressed medially by the triceps muscle and bent toward the cubital tunnel. The Osborn ligament was intact. The clinical diagnosis was severe ulnar neuropathy and ulnar nerve decompression was performed. Ulnar nerve entrapment at the ligamentum epitrochleo-anconeus was confirmed during surgery.
